Behavioral Health Consultant (Licensed Professional Counselor/Licensed Clinical Social Worker) - UVA Augusta Pediatrics/UVA Culpeper Pediatrics
UVA Augusta Pediatrics/Culpeper Pediatrics is seeking a Licensed Professional Counselor or Licensed Clinical Social Worker to join their team as a Behavioral Health Consultant as they establish integrated behavioral health services in their practice in Fishersville, VA/Culpeper, VA. The Behavioral Health Consultant will work as a member of the primary care team, providing consultation-based services to patients in collaboration with their primary care providers. Responsibilities include:
- Providing access to focused assessment and appropriate collaborative care and treatment in the primary care setting, including regular same-day consultation visits (i.e. “warm handoffs”).
- Delivering evidence-based interventions to patients experiencing mental health symptoms, behavioral health issues, as well as for the promotion of general health.
- Practicing collaborative, stepped and measurement-based care, including appropriate follow-up, that reflects a population health-influenced approach for the primary care population.
- Collaborating with primary care team members to develop clinical pathways for select priority populations (e.g. behavioral parenting classes for families of patients with ADHD).
- Contributing to the optimal functioning of primary care teams through collaborative decision support and interdisciplinary consultation that is mindful of other team members’ roles and workflows.
- Assisting with care coordination and access to outside resources for patients as appropriate.
- Participating in any relevant clinical training, quality improvement activities, and staff meetings.
Desired experience and skills:
- Experience with providing clinical interventions to pediatric populations.
- Comfort with individual, parent-focused, and family-focused interventions.
- Interest in working in a fast-paced primary care setting and as part of an interdisciplinary team.
- Willingness to maintain flexibility in the clinic schedule to adapt to the needs of the primary care team.
