The LPN's primary role is to support patients with chronic conditions understand and manage their disease.
This position follows a hybrid schedule with three in-office days per week.
Primary Responsibilities:
- Deliver exceptional customer service and maintain established quality control standards
- Help patients understand their assigned care plans and provide outreach to those who are not engaging
- Redirect patients to Primary Care as needed to ensure comprehensive care
- Stay up to date on current care plans and expansions
- Maintain accurate patient records and data
- Support patients in understanding and managing their chronic conditions, promoting better health outcomes

Required Qualifications:
- Graduation from an accredited school of nursing
- Active Licensed Practical Nurse license through the Washington State Department of Health
- Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross)
Preferred Qualification:
- 2+ years of experience working as an LPN
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
